Clinical Pharmacist – Community (Full-time, Lagos)

About the role

The Clinical Pharmacist – Community role is a full‑time, on‑site position based in Yaba, Lagos State, Nigeria. The incumbent will deliver clinical and community pharmacy services, ensuring safe and effective medication use while promoting rational drug therapy.

Key responsibilities

• Review prescriptions for appropriateness, drug interactions and contraindications.
• Monitor and manage medication therapy, including medication therapy management and pharmaceutical care planning.
• Collaborate with prescribers and multidisciplinary teams to optimise patient outcomes.
• Assist with compounding and safe preparation of medications when required.
• Ensure compliance with regulatory, safety and quality standards.
• Maintain accurate patient and dispensing records.
• Contribute to patient education initiatives and counseling.
• Support inventory management to guarantee availability of essential medicines.

Required profile

• Strong knowledge and practical skills in Pharmacy Practice and Pharmaceutical Care.
• Solid understanding of pharmacy and medicine in both clinical and community settings.
• Competence in compounding and safe medication preparation.
• Bachelor of Pharmacy (B.Pharm) or Doctor of Pharmacy (Pharm.D) with current licensure from the Pharmacists Council of Nigeria.
• Experience in community or hospital pharmacy practice with a focus on patient‑centred care and medication safety.
• Excellent communication and counseling abilities, capable of working in multidisciplinary teams.
• Attention to detail, ethical conduct and adherence to professional standards.
• Proficiency in basic health IT systems for prescription processing, documentation and inventory management.
