Community Health Worker

About the role

The Community Health Worker will lead outreach initiatives to improve health awareness and preventive care within local communities. You will educate individuals and families on hygiene, nutrition, maternal and child health, immunisation, and healthy lifestyles while supporting access to health services.

Key responsibilities

• Conduct community outreach programs and health education sessions.
• Identify health needs, provide guidance, and refer clients to appropriate services.
• Organise and participate in medical outreaches, vaccination drives, and wellness screenings.
• Perform basic health assessments such as blood pressure, glucose, temperature, and weight measurements.
• Visit homes to monitor health status and reinforce education.
• Maintain accurate records of interactions, referrals, and community health reports.
• Collaborate with healthcare professionals and stakeholders to ensure continuity of care.
• Report disease outbreaks or public health concerns to authorities.

Required profile

• Minimum OND in Community Health, Public Health, Nursing, Health Education, Environmental Health, or related field.
• At least 1 year of relevant experience in community or primary health programs.
• Valid professional certification or licence is an advantage.
• Strong communication, interpersonal, and counselling abilities.
• Ability to work independently and as part of a multidisciplinary team.
